The 5x5 grid pays via matching clusters of four or more symbols connected horizontally or vertically, with winning clusters cleared from the board and new symbols cascading down to refill the gaps until no further clusters form.

Four skull icons make up the low-value symbols, while spiders, crosses, snakes, and hearts sit above them as the premiums; the largest clusters, at fifteen or more matching symbols, pay between 40x and 200x the stake. A regular Tome wild substitutes for any standard symbol, and a second, stacked Alice Cooper Mega Wild plays a separate role in the feature set described below.

On any spin that doesn't produce a win, one of four Reality Effects modifiers can trigger at random: Chamber of Horrors removes symbols outright and drops wilds into the gaps left behind, Temptation Room adds a wild while also converting other symbols to match it, The Dungeon turns the low pays into varying premiums, and Insane Asylum turns every premium symbol on the grid into a single matching one. Separately, key symbols can land on the reels and leave a wild behind once collected; gathering three keys in one spin summons the stacked Mega Wild onto reel two, three, or four, where it walks across the grid between cascades before eventually walking off.

Every symbol cleared in a cascade also charges a meter called the Hat of Madness, and hitting set thresholds unlocks different rewards: ten symbols adds one or two wilds to the next cascade, twenty-five adds three to six, fifty awards three Reality Spins outright, seventy-five awards three Reality Spins carrying a 2x multiplier, and eighty-five awards three Reality Spins at 5x. Only the highest threshold reached in a sequence pays out, and reaching the Reality Spins round hands players a choice of which Reality Effect plays out at the end of each spin within it. Should the Mega Wild appear and walk during Reality Spins, there's a chance it drops three to six additional wilds onto the grid as it goes.
