Pragmatic Play sets Eye of Cleopatra on a 5-reel, 4-row grid, pairing the queen's imagery with a wild-generating device positioned to the right of the reels. Columned Egyptian architecture fills the background, with a matching soundtrack running throughout.

Symbols pay when three or more matching icons land across any of the game's 20 paylines from the leftmost reel. Four low-value symbols are J-A royals, worth 0.25x to 1x the stake for five of a kind, while five higher-paying icons — snakes, cats, birds, a pharaoh, and Cleopatra herself — return 1.25x to 10x. Wilds can land in any position and substitute for everything except the scarab scatter.

The wild pattern device is the game's central mechanic: a golden inner circle represents the wild positions on the reels, surrounded by eight outer positions that can light up at random on any spin. When a lit outer position sits next to a wild already on the reels, the adjacent symbol also converts to wild before wins are counted, so a single spin can chain several transformations together if the wilds and lit positions line up.
