A soundtrack borrowed from the same folk tune that has followed every version of Tetris found its way into Tetris Extreme too, the block-stacking slot Red7 built for SG Digital's platform around a falling-block mechanic rather than a standard reel spin. Stakes run from 0.20 up to 100, cluster pays replace a fixed payline structure, and volatility is rated medium-to-high, leaning toward the upper end of that band.

Instead of a hand of static reels, each spin drops coloured blocks steadily until the entire play area fills. Completed horizontal rows clear away to make space for further blocks, and every row cleared nudges a multiplier meter upward, capable of reaching x50 purely within ordinary spins. Once no more rows can be completed, the game looks for a group of nine or more matching blocks left standing; find one, and whatever the meter has built up applies to that payout. Colour makes no difference to value — a cluster of 25 or more blocks alone returns 8x the stake before any multiplier lands on top. Because a single drop can trigger several consecutive row clears before the grid settles, individual spins can take noticeably longer to resolve than a standard reel-based game.

Clearing fifteen or more rows in one drop unlocks ten free spins, played under the same falling-block rules with one addition: a golden block variant can appear mid-round, and folding it into a row clear multiplies that clear's own multiplier by a further five on top of anything already accumulated.

A linked jackpot version called Mega Drop also exists, funded by two percent of every wager and distributing three separate prizes — Minor, Major and Epic — awarded at random with their own visible countdowns.
